You will be required to sign a contract to adopt a Mill Dog Rescue Network dog and it requires the adopter to be bound by and comply with all of the following terms and conditions.  A Mill Dog Rescue Volunteer will go over the contract with you in person at the time of the adoption.

Mill Dog Rescue Network  Adoption Contract

1.	I will provide proper and sufficient food, water, shelter and kind treatment for the dog at all times.  The dog will never be kept primarily outside.
2.	I will have the dog immunized and examined annually, at my expense.
3.	I will, if the dog is not already spay/neutered at the time of adoption, have it spayed or neutered as arranged and paid for by the Mill Dog Rescue Network by a specified date.  In the event I fail to have the dog spayed or neutered within said time, I understand and agree that I will forfeit the adoption fee and I agree that I will, upon demand, immediately return the dog to the Mill Dog Rescue Network.
4.	I will provide an ID tag with my contact information on it to be worn by the dog at all times.
5.	I will not sell, abandon or give away this dog and agree that if I no longer desire to retain the dog, I will return it to Mill Dog Rescue Network.
6.	I will agree that a representative of Mill Dog Rescue Network may periodically wish to see the dog, my residence and the living conditions which I provide for the dog, and I further agree to surrender the dog to a representative of Mill Dog Rescue Network if for any reason he/she considers the conditions provided by me for the dog unsuitable.
7.	I will not permit the dog to run at large or to become a public nuisance.  I also agree to immediately retrieve the dog at my expense from any public pound or dog shelter when notified that the dog is being held at any such location.  In the event the dog becomes lost, I will make every reasonable effort and attempt to relocate and claim the dog.
8.	I will notify Mill Dog Rescue Network if the dog is lost and not found or passes away for any reason.
9.	I agree that I will at no time assert any claim, charge or demand of any kind against Mill Dog Rescue Network for any expenses that may have been incurred by me, including veterinary fees, in connection with this dog.  I also agree that any illnesses or injury occurring after adoption will be my sole responsibility.
10.	I hereby accept the dog as is, assume all risk of ownership of the dog, including the risk of injury or damage caused by the dog.

This contract may be specifically enforced by Mill Dog Rescue Network through judicial proceedings.  I hereby agree that in the event I breach this contract and Mill Dog Rescue Network files suit to enforce this contract or to defend any claims under this contract, that I will pay any court costs and attorney’s fees incurred by the Mill Dog Rescue Network in connection herewith.
